| Who We Are |
Utah Partners for Health is a nonprofit charitable organization that provides funding for the provision of medical and ancillary services to the uninsured, underinsured, and underserved in Salt Lake County. Our challenge is to deliver access to healthcare to those who do not qualify for federal or state programs or are otherwise denied access to care within their communities. |

| Our Mission |
Our mission is to improve access to healthcare for those in need. This growing population consists of young families, single parents, the elderly, immigrants and other medically underserved individuals within Salt Lake County. In many cases they are our neighbors, friends, and relatives who are caught in the gap of not having insurance and therefore avoid seeking healthcare.
I am married with two children, one with asthma. My husband and I both work 40 hours a week making minimum wage by washing dishes in a restaurant. We don't qualify for Medicaid because we make too much money. My son and I came in yesterday and he was having obvious trouble with his asthma. We were able to get my son immediate medical care, including breathing treatments. Thank you Utah Partners for Health! - Karen, 32-year-old |

| Our Challenge |
The facts are clear about the uninsured. They get sicker for longer periods and die younger. A survey of physicians on the front lines was released showing that uninsured patients are more likely to live with serious medical conditions, lack access to critical medications and forgo recommended health screenings than people who have health insurance. This prolongs sickness, complicates conditions, and can cause premature death. Another simple fact is when health care is denied it leads to problems later in life.
"Every single day I work at the clinic, I see patients who put off getting care for a long time because they can't afford it. So a condition that could have been controlled develops into something much more serious, something more difficult, and even more expensive to treat." - Family Physician |

| Healthy Communities |
| Healthy communities are a competitive advantage. Forging partnerships will make a difference in society. We will be stronger for the involvement of all who help build good schools, decent neighborhoods, work that dignifies, and cohesive communities. |
| |
| Our Model |
| Our healthcare model is unique because we work within for-profit healthcare clinics and therefore do not have to pay for buildings, supplies, utilities, or medical staff. We are then able to leverage our donations to more than $5.91 in healthcare provided for every $1.00 donated to UPFH. Our healthcare clinics are open seven days a week and allow access to care on a same day or next day basis. |
