| Who We Are |
| Utah Partners for Health is a nonprofit charitable organization that provides funding for the provision of medical and ancillary services to the uninsured, underinsured, and underserved in Salt Lake County. Our challenge is to deliver access to healthcare to those who do not qualify for federal or state programs or are otherwise denied access to care within their communities. |

| Our Mission |
| Our mission is to improve access to healthcare for those in need. This growing population consists of young families, single parents, the elderly, immigrants and other medically underserved individuals within Salt Lake County. In many cases they are our neighbors, friends, and relatives who are caught in the gap of not having insurance and therefore avoid seeking healthcare. |

| Our Model |
| Our healthcare model is unique because we work within for-profit healthcare clinics and therefore do not have to pay for buildings, supplies, utilities, or medical staff. We are then able to leverage our donations to more than $5.91 in healthcare provided for every $1.00 donated to UPFH. Our healthcare clinics are open seven days a week and allow access to care on a same day or next day basis. |

| Our Providers |
| The licensed healthcare providers that provide services funded by Utah Partners for Health are some of the best healthcare providers in Utah. They care about the community and volunteer to provide charitable care throughout the year. Our providers donate an average of 67% of their customary billing charges to Utah Partners for Health to allow for the greatest benefit from every dollar donated to the organization. |

| Our Services |
| Utah Partners for Health currently offers access to primary healthcare services for acute, chronic, and mental illnesses. Ancillary services such as lab work, radiology, ultrasound, echocardiogram, and physical therapy are also available. Other services including counseling, pharmaceutical, dental, and eye care are provided for those who qualify. UPFH also offers a prenatal clinic and mobile medical unit to reach those who do not access to medical services within their own communities or are denied access to healthcare for other reasons. The community impact that UPFH has in Salt Lake County is remarkable, with an average growth in patients seen each year at 75% annually.
