| Utah Partners for Health is a 501c3 charitable organization that provides funding for the provision of medical and ancillary services to the underserved in Magna, West Valley City, and Kearns. |

| Our Mission |
| Our mission is to improve access to healthcare for those in need. Our challenge is to deliver health care to the growing number of underserved and uninsured who do not qualify for federal or state health care programs. This growing population consists of young families, single parents, the elderly, immigrants and other medically underserved individuals. In many cases they are our neighbors, friends, and relatives who are caught in the gap of not having insurance and therefore avoid seeking healthcare. |

| Our Challenge |
The facts are clear about the uninsured. They get sicker for longer periods and die younger. A survey of physicians on the front lines was released showing that uninsured patients are more likely to live with serious medical conditions, lack access to critical medications and forgo recommended health screenings than people who have health insurance. This prolongs sickness, complicates conditions, and can cause premature death. Another simple fact is when health care is denied it leads to problems later in life.

| Our Model |
| Our healthcare model is unique because we work within for-profit healthcare clinics, and therefore do not have to pay for buildings, supplies, utilities, or staff. We are able to leverage our donations to equal over $3.50 in healthcare provided for every $1.00 donated. Our healthcare clinics are open seven days a week and allow access to care on a same day or next day basis. |

| The licensed healthcare providers that will provide services funded by Utah Partners for Health are some of the best healthcare providers in Utah. They care about the community and are willing to provide charitable care. Our providers have been donating an average of 61% of their customary billing to Utah Partners for Health. |

| Our supporting partners recognize their leadership role in contributing to the common good of the community. Corporations, small businesses, agencies, churches, community organizations, professionals, and individuals have joined an alliance that creates diverse benefits for both the partners and the community. |

| Our Services |
| Utah Partners for Health operates within six clinics, offering primary healthcare services for acute, chronic, and mental illnesses. Ancillary services such as lab work, radiology, ultrasound, echocardiogram, and physical therapy, also are available. Other services, including counseling, pharmaceutical, dental, and eye care services are provided for those who qualify. |

Our Care |
All decisions made by our healthcare providers in the delivery of care and services are made in the patient's best interest with a respect for the dignity, privacy, desires, culture, and choices of the patient and family. Services are appropriate, flexible, and sufficient in duration and scope to meet the needs of each patient.
Individuals in the medically underserved community who receive charitable medical care through funds provided by Utah Partners for Health will be encouraged to contribute to their healthcare to the extent they are able. The small co-pay required is a component of our model and allows others in the community to be served. |

| Eligibility For Care |
Utah Partners for Health will limit its benefits to individuals who comprise low income and medically underserved residents in Magna and surrounding communities. The "underserved community" are those individuals in need who cannot afford medical care covered by traditional insurance carriers.

| Managing our Funds |
We deliver significant social impact per dollar donated. The Utah Partners for Health goal is to deliver a minimum of $3.35 in patient health care for every $1.00 donated. We currently are delivering $4.61 in patient healthcare for every $1.00 donated.
This is accomplished through the charitable partnerships of our providers with the vast supporting network that cares for the Magna and surrounding communities. |

| The Uninsured |
Approximately 412,000 residents are without health insurance at some point during each year. Nine out of ten are families with working adults. People without health coverage are at risk for health and financial catastrophes. The United Way 2007 Community Assessment designated insufficient income and lack of affordable health care as two of the most serious issues facing Salt Lake County residents. Some of the causes identified included lack of early treatment leading to escalated problems, cultural/language barriers, and rising costs. Trends identified as "worse" include diabetes, hypertension, and obesity. Small businesses dropping insurance benefits are also increasing. The assessment noted that Utah is one of nine states nationally that experienced a significant increase in poverty rates.
The Utah Department of Health also revealed the severity of the need with their health status report. The cost of health care has increased and the high cost of premiums have caused employees to decline coverage. |
