| Who We Are |
Utah Partners for Health is a nonprofit charitable organization providing funding for the provision of medical and ancillary services to the uninsured, underinsured and underserved in Salt Lake County. Our challenge is to deliver access to healthcare to those who do not qualify for federal or state programs and are denied access to care in our community. |

| Our Mission |
Our mission is to provide access to healthcare for those in need. In Utah that number continues to grow and in many cases our loved ones fall captive to high medical costs and thousands live the risk of life without insurance.
I am married with two children, one with asthma. My husband and I both work 40 hours a week making minimum wage by washing dishes in a restaurant. We don't qualify for Medicaid because we make too much money. My son and I came in yesterday and he was having obvious trouble with his asthma. We were able to get my son immediate medical care, including breathing treatments. Thank you Utah Partners for Health! - Karen, 32-year-old |

| Our Challenge |
The facts about the uninsured are clear: they are sick longer and die younger. Studies continue to demonstrate that those who are uninsured are less likely to receive preventative care and therefore suffer the serious consequence of untreated disease or forgoing recommended health screenings. An estimated 22,000 adults between the ages of 25 and 64 die prematurely each year for not having each year for not having access to care because they are uninsured.
"Every single day I work at the clinic, I see patients who put off getting care for a long time because they can't afford it. So a condition that could have been controlled develops into something much more serious, something more difficult, and even more expensive to treat." - Family Physician |

| Healthy Communities |
UPFH understands the importance of a healthy community and have crafted a program that offers full access to primary care services for the uninsured. We help pregnant mothers with prenatal visits to ensure a healthy delivery for them and their children. We offer services so that children receive well care checks and immunizations to attend school, or so when they grow up they can receive physicals or care if they get the flu.
In Utah, lack of affordable healthcare is a top concern and one of the leading causes of bankruptcy for working families. With the faltering economy, the number of uninsured and jobless residents is rapidly increasing. Many of the working poor have lost access to coverage through jobs or state/federal programs. At conservative estimates, the number of uninsured residents in Utah is about 350,000. Nine out of ten of these families have at least one working adult, but do not have access to affordable insurance through the workplace.
For communities that can't access our services in-clinic, our partnered mobile clinic brings access to care directly to them with our mobile events.

| Our Model |
| Our unique and innovative access model is possible because we work within for-profit healthcare clinics. We, therefore, do not pay for a building, supplies, or staffing, and 95 percent of funding goes directly to services. We leverage more than $5.93 in healthcare for every $1.00 donated, and our healthcare clinics are open seven days a week and allow access to care on a same day or next day basis. |
