Our Services
Utah Partners for Health currently offers access to primary healthcare services for acute, chronic, and mental illnesses. Ancillary services such as lab work, radiology, ultrasound, pulmonology, and physical therapy are also available. Other services including counseling, pharmaceutical, dental, and eye care are provided for those who qualify. See below for more information about our access to prenatal care services and access via our partnering mobile medical clinic.

Our healthcare clinics are open seven days a week and allow access to care on a same day or next day basis. The community impact that UPFH has in Salt Lake County is remarkable, with an average growth in patients seen each year at about 75% annually.
 
Our Care
The licensed healthcare providers who provide services funded by Utah Partners for Health are some of the best healthcare providers in Utah. Our providers donate an average of 67% of their customary billing charges to Utah Partners for Health to allow for the greatest benefit from every dollar donated to the organization.

All decisions made by our healthcare providers in the delivery of care and services are made in the patient's best interest with a respect for the dignity, privacy, desires, culture, and choices of the patient and family. Services are appropriate, flexible, and sufficient in duration and scope to meet the needs of each patient.

Individuals in the medically underserved community who receive charitable medical care through funds provided by Utah Partners for Health will be encouraged to contribute to their healthcare to the extent they are able. The small co-pay required is a component of our model and allows others in the community to be served.
 
Eligibility For Care
Utah Partners for Health will limit its benefits to individuals who comprise low income and medically underserved residents in Magna and surrounding communities. Our mobile medical unit also provides access to healthcare to anyone residing in Salt Lake County and Wendover, Utah. The "underserved community" are those individuals in need who cannot afford medical care covered by traditional insurance carriers.

What we offer
The mission of Utah Partners for Health is to deliver access to healthcare to the growing number of underserved and uninsured who do not qualify for federal or state healthcare programs. This growing population consists of young families, single parents, the elderly, immigrants, and other medically underserved individuals. In many cases they are our neighbors, friends, and relatives who are caught in the gap of not having insurance and therefore avoid seeking healthcare.
 
Qualifications:
  • Currently uninsured or underinsured.
  • Live within the cities of Magna, West Valley City or Kearns, Utah for access to care at our clinic locations.
  • Live within the State of Utah for access to care through our mobile medical unit.
  • Live within the State of Utah for access to our prenatal program.
  • A household income of less than 55% of median income.
  • Sick with a current medical need.
  • Other limitations may apply.
Utah Partners for Health provides access to care for the following medical needs:
  • Family Practice Medical care
    • Acute care - illness, infection, fractures, lacerations, etc.
    • Chronic illness care – diabetes, hypertension, etc.
    • Mental illness care – bipolar, ADHD, depression, anxiety, etc.
    • Preventive Care - clinical breast exam, cervical exams, tobacco cessation, etc.
    • Well child checks for students who need an exam to attend school.
  • Counseling Services
  • Dental Care
    • Urgent dental needs for elementary students
  • Eye Care
    • Medical referrals to an eye doctor
    • Vision care for students
  • Ancillary Services
    • DXA Scans
    • Physical Therapy
    • Lab Services
    • Ultra Sound
    • Echocardiogram
  • Pharmaceutical Assistance
  • Referral Service

Costs to the Patient - Individuals who receive charitable care are encouraged to pay a small portion for the services rendered. The small co-pay is required so that others in the community can be served.
  • Existing Patients will be required to pay approximately $25 or more depending on the service rendered.
  • New Patients will be required to pay approximately $35 or more depending upon the service rendered.
  • Dental and Eye Care patient costs are higher depending on the procedures needed.
  • Exceptions can be made depending upon the needs of the patient and funds available.
 
Prenatal Clinic
Utah Partners for Health offers prenatal care, case management services, lab work, and post-partum care to any woman living in Utah who qualify for services. Hundreds of women who otherwise do not have access to prenatal services within these communities are served each year by excellent medical staff who want to ensure the health of every mother and baby in our community.
 
Mobile Medical Unit
Utah Partners for Health funds a Mobile Medical Unit that travels around Salt Lake County neighborhoods and housing areas to give our providers access to uninsured individuals who are in desperate need of primary healthcare services. Education, screening, and treatment referrals are performed for hundreds of families and individuals through the mobile medical unit each year.

Or you can contact us at (801) 801-417-0131 and we will ask you the questions on the form below by phone.

To be eligible for aid at our medical clinic locations you must live in the cities of Magna, West Valley or Kearns. To be eligible for aid through our Mobile Medical Unit you must live in Salt Lake County. To be eligible for aid through our prenatal clinic you must live within the State of Utah.

Applicants who qualify for Medicaid will be referred. Acceptance is based on, but not limited to, the availability of funds, medical need, and the income of the applicant. Under most circumstances we will let you know within two business days the results of your application.
